She was said to have gained great influence over Varian after the death of Queen Tiffin Ellerian Wrynn, during which he subsequently fell into a deep depression. King Varian eventually beat his depression and started to reassert his control over the Stormwind government.
After King Varian Wrynn went missing under suspicious circumstances while en route to a diplomatic summit to Theramore Isle, Stormwind was believed to be going through a state of disarray. Young Anduin was given the crown so that order could be preserved within the kingdom of Stormwind, at the behest of the royal councilor, Lady Prestor. Highlord Bolvar Fordragon acted as a kind of Regent of Stormwind or the Supreme Commander of Stormwind's forces on behalf of King Anduin. When Magistrate Solomon gave warnings of Blackrock orcs allying with the Black dragonflight, Bolvar was debilitated from sending Stormwind's thinly spread troops to aid Lakeshire without proof. Bolvar sent Alliance adventurers to Lady Prestor, who knew much of dragons and dragonkin. However, Lady Prestor thought very little of Magistrate Solomon's claims and advised against providing the military aid. Though Bolvar respected Lady Prestor's stance, he could not let Solomon's plight go unheeded and thought of a compromise to appease all parties: Bolvar deputized the Alliance adventurer and empowered them to solve Solomon's problems.
When Lady Jaina Proudmoore found evidence of the Defias Brotherhood being involved in King Varian's disappearance and had a powerful ally in Stormwind, she sent the evidence to Bolvar. With their newfound patronage and apparent role in the king's disappearance, Bolvar saw no choice but to plan a renewed offensive against the Defias. While Bolvar sought to pursue the Defias and their backers until King Varian's return, Lady Prestor tried to belittle the issue as just baseless speculation and a waste of Stormwind's resources.
King Varian was eventually ransomed back to Stormwind by the naga, a ransom that was paid with a painful new tax. The king that returned however, became befuddled around Lady Katrana and permitted her to prevent Stormwind from reinforcing their allies: the Ironforge dwarves, in the retaking of Thandol Span.
She controlled Highlord Bolvar through the use of the . She manipulated the leaders of the Kingdom of Stormwind and the Defias Brotherhood through politics and in some cases, mind control. She has been keeping them in the dark while her brother Nefarian — Lord Victor Nefarius — manipulates the Dark Horde in its battle against Ragnaros.
King Varian's disappearance was a closely-kept secret, and his sudden "return" (later proven to be a weaker version of Varian that Lady Prestor could use as a puppet ruler) was treated with skepticism by both Prince Anduin and other leaders of the Alliance such as King Magni Bronzebeard. Meanwhile, Lady Katrana has sent assassins to assassinate a gladiator named Lo'Gosh during his journey to Stormwind.
Eventually, it is revealed that Katrana Prestor was the mastermind behind King Varian's kidnapping. Katrana hired the Defias Brotherhood to kidnap Varian during his journey to Theramore and had him brought to Alcaz Island. It was there that Katrana and her dragonspawn minions conducted a ritual that split King Varian into two people, with the intention of murdering the Varian that embodied Varian's strong will. Before Katrana could complete her ritual however, she and her forces were attacked by naga led by Morgala Darksquall. Eventually, the kingdom of Stormwind ransomed back the weak willed Varian and Katrana ensorcelled that Varian to act as a befuddled figurehead while she ran Stormwind from behind the scenes. The strong willed Varian eventually escaped his naga captors and got amnesia. The amnesiac Varian was enslaved by a Horde gladiatorial trainer in Kalimdor and fought his way to freedom, earning the nickname "Lo'Gosh," or "ghost wolf," while in battle.
When Lady Prestor was revealed to be the broodmother Onyxia by Marshal Windsor, whom she killed on spot, Onyxia kidnapped Anduin and held him hostage in her lair. King Varian's two halves and a band of Alliance heroes traveled to Kalimdor and rescued Anduin from the black dragon and at long last, brought Onyxia to justice. During the battle at Onyxia's Lair, the two Varians botched Onyxia's spell to destroy Lo'Gosh and fused together, returning King Varian to his original form.
Lady Prestor is the human form of the black dragon Onyxia, broodmother of the black dragonflight and daughter of Deathwing. Her name is a homage to the form her father took when human, Lord Daval Prestor though their relation, if any, is unknown. While Nefarian is her brother, there is no evidence that the Victor Nefarius' human identity is supposed to be related to the "Prestor" line.

In the RPG
Lady Katrana Prestor and Lord Bolvar Fordragon serve as the king's main advisors, and they pretty much call the shots. However, Bolvar is not one to insult the judgment of an "educated" lady like Katrana Prestor. It seems like her advice is pretty much the exact opposite of anything a logical person with half a brain would come up with. As a result of Lady Prestor's intervention, proper human leadership has become impossible. She doesn't seem to think the other races are necessary. Katrana won't even allow the king to send reinforcements to human lands that desperately need it, such as Duskwood and the Redridge Mountains. Every report of enemy activity on those fronts is shot down for lack of "proof", despite knowledge of dead men and women. It's unknown what she did to get the prince's ear, but for whatever reason she’s managed to keep both the king and Lord Bolvar following her directions (and some of them are clearly nonsense). She's just playing games with the Alliance troops when they have good men and women dying out on the front. Her motivations are unknown; she's certainly either working for something malicious, or she is malicious herself.

- Katrana's name may be derived from the word katran (катран in the Cyrillic script) present in several Slavic languages, meaning tar. If this is the case, her name refers to her real identity as a black dragon.
Daval Prestor II, a second "Lord Prestor", appeared in the World of Warcraft Beta, but was then replaced with Katrana. Katrana's exact relation to the original Daval Prestor is unclear, though it's presumed that she is his daughter - just as Onyxia is the daughter of Deathwing.
