The Maw of Madness is a subzone of the Twilight Highlands where Iso'rath dwells. The surrounding landscape is defiled and filled with Iso'rath's tentacles, in addition to numerous Faceless Ones.
Trivia
The name is most likely a version of 1995 film "In the Mouth of Madness", which was heavy with Lovecraftian inspiration, especially the book "At the Mountains of Madness". Lovecraft is also believed to have been the inspiration for most of the names of the Old Gods in World of Warcraft.
